We require prepayment and patient information to schedule the appointment. Our virtual office accepts PayPal and all valid credit cards through PayPal Payments. Please fill out the following information so that we can book your appointment and process payment. Once payment has been successfully processed, you can either complete the required Health History Forms and the Patient Acknowledgement of Telehealth Consultation Services immediately after or return later to complete them prior to your teledental appointment.

You will receive an e-mail confirmation of this appointment and a unique link that will allow you to view this appointment and provide any updated information. If this unique link and receipt does not arrive via e-mail within 2 hours after payment is processed, please contact us by office@calladentalsolutions.com immediately.

IMPORTANT NOTE: This teledental visit does not replace a bi-annual dental visit. By clicking the button to tender payment and schedule your appointment, you certify to us that you understand that you are scheduling a teledental session for us to review any dental related topic/question/inquiry that you may have, and that you understand that this session is not a direct replacement for a bi-annual routine preventative dental visit. You further understand that teledental services, once scheduled successfully, are non-refundable. If you are covered with dental insurance, we will provide you any documentation you may require to have the amount you are entitled to paid to you directly by your insurer, however, we still require full prepayment before we will see you. Your insurer will pay you directly what benefits you are entitled to, as the contract is between your insurer and you, not us. Thank you for your understanding and we look forward to treating you.
